COVID-19 Office Updates

June 18th, 2020

At Optometry Corner, our focus has always been on providing safe and effective eye care and vision care to the best of our abilities. As we resume eye care in our office, the health and safety of our patients and their family members remains our top priority.

Our office has recently implemented health and safety protocols based on the guidelines set forth by the Centers for Disease Control and Prevention, https://www.cdc.gov/coronavirus/2019-ncov/prevent-getting-sick/prevention.html, and the American Optometric Association, https://www.aoa.org/coronavirus

As part of these guidelines to help minimize the spread of COVID-19, we highly recommend all patients to wear a face covering when visiting our office, and our staff will be measuring your temperature when you arrive. Please reschedule your appointment if you have traveled within the last month and/or if you are experiencing any flu-like symptoms.

To help reduce wait times during your appointment, please complete this Wellness Form before your visit: https://book.getweave.com/1337c924-c146-4645-b59c-6eb85a4b3ba9/wellness

If you are new to our office, please also complete this Patient Registration Form online before you arrive: https://www.optometrycorner.com/contact-us/patient-registration-form/

These health and safety protocols are in place to continue keeping our patients, our doctors, and our healthcare team safe during these unprecedented times. Please let us know if you have any questions. We're here to help!

Hope to see you soon,

Optometry Corner Team
